Pre-Examination Patient Information

Knowing your insurance and vision plan benefits ahead of time saves you a lot of time and hassle during your visit.  Health insurance and vision insurance are not always the same thing.  Most health insurances, such as BlueCross BlueShield, have a seperate vision plan such as VSP or Eyemed.  If you're coming in for a medical visit, we'll use your medical insurance.  If you're coming in for a vision exam, glasses, and/or contacts, we'll use your vision insurance.  Ask your employer or your human resource department what your vision plan and plan ID# are prior to your visit.

Try to bring in your current pair of glasses and your contact lens boxes if you have them.

If you're getting contact lenses, it's okay to come in wearing your current pair.

Bring in your list of medications unless you fill out the check-in forms ahead of time.

Our opticians love helping you choose the right frame for the look you want.  However, you may want a family member or friend with you to give their opinion as well.

Please have the financially responsible person come in to the exam with the patient.  It is important for the financially responsible person to understand any eye conditions and treatment options.

We rarely run more than 20 minutes behind, but if we do we do we'll post it on our Appointments/Wait Time page.

We have television for those coming with you.  They can change the channels and volume at their will.

We realize it is sometimes necessary to bring small babies.  Our staff will assist you as much as possible so you can give your full attention to the eye exam.

Consider filling out the three check-in forms ahead of time and bring them in (or e-mail them) to us.  This will save you time.

Allow yourself enough time to check-in, be seen for your exam, fit contacts, pick out glasses, and be dilated.  While the process can take as short as 45 minutes, allow up to two hours total so you can be as relaxed as possible during the process.

If you're under time constraints, please let the receptionist and doctor know what time you have to leave by.

The front desk will let the doctor know you are ready via Skype.

If follow-ups will be necessary, bring your calender so you can schedule the next visit.

Most vision plans pay for basic materials and can be used towards the cost of either glasses or contacts, but not both. Most patients want more than just the very basic materials or features. Contact lens exams cost more than glasses only exams.  We do have additional vision coverage or Care Credit available.  We require at least half down before starting an order.

The patient's portion of the bill must be paid in full before the prescription for eyeglasses, contact lenses, or medicine is released.

Eyeliner can sometimes smudge a little during a contact lens exam.  Consider keeping the eyeliner as light as possible.

Most patients are dilated after they select their eyeglass frames.  You'll be able to drive afterwards if you have a current pair of glasses.  However, it will be difficult to read up close through your distance glasses or contacts for about 3 hours.  You will be light sensitive for about 3 hours afterward, so if you don't have dark sunglasses we'll give you a temporary pair to slip behind your glasses or wear over your contacts.  Consider bringing a driver if you usually don't see well after dilation.

Sometimes it is necessary for the doctor to take photos of something in the eye that has potential to worsen.  Your health insurance will be billed for these photos.  Deductibles and co-pays may apply.

MapQuest or GPS often erroneously puts our location at Dobson and Pecos.  We are actually at Cooper and Pecos.  If you use GPS, consider putting in these crossroads rather than our exact address.

We have handicap capable bathrooms and exam rooms. However, handicap parking is around the side of the building.

Most parents feel the inherent desire to coach their child through the eye exam.   Our doctor has worked with children for years and knows how to gently improve cooperation.  For some reason, children often cooperate better in such circumstances when the parent is not in the room.  Don't be offended if the doctor asks you if you would be willing to wait outside for a few moments.

Usually orders on eyeglasses take about a week to a week and a half, depending on the features you desire.  We can finish some jobs the same day.  Delays occur if the work does not meet our high standards for quality.  We will do everything we can to notify you if there is a delay.  We apologize for any inconvenience.  Let us know if you have a deadline for getting your glasses.

Unfortunately, we cannot dispense the glasses or contact lenses until they are paid for in full. Sorry for any inconvenience. We have been nice in the past but too many times people have taken advantage of us. Please understand we have to pay vendors for the products when they are ordered.

If you have any questions, please don't hesitate to call us at 480-812-2211.  WE LOOK FORWARD TO SEEING YOU!
